چکیده

Objective: This study aims to analyze the complications of endoscopic transsphenoidal surgery for pituitary neoplasms. Methods: 326 patients who underwent neuroendoscopic endonasal transsphenoidal surgery at the neurosurgery department of the First Affiliated Hospital of Chongqing Medical University from July 2012 to July 2015 were enrolled in this retrospective analysis. Results: Among the 326 patients, the cases of postoperative hyponatremia, diabetes insipidus, hypopituitarism, postoperative cerebrospinal fluid leak, meningitis, cranial nerves palsy, epistaxis, sphenoid sinusitis, internal carotid artery injury and residual tumor bleeding respectively are 32 (9.8%), 12 (3.7%), 23 (7.1%), 13 (4.0%), 1 (0.3%), 3 (0.9%), 5 (1.5%), 3 (0.9%), 1 (0.3%) and 1 (0.3%). Conclusion: Endoscopic endonasal transsphenoidal surgery has the considerable total resection rates for pituitary adenomas and preventable postoperative complications.

Endoscopic Endonasal Transsphenoidal Approach for Pituitary Adenomas: a Prospective Review of Our Early Experience

Background & Aim: Pituitary adenomas are a part of a dissimilar group of benign neoplasms. The development of endoscopic techniques for surgery of paranasal sinuses has increased the opportunity for an endoscopic approach with regard to the pituitary gland. Minimally invasive endoscopic pituitary surgery, in turn, permits a more thorough tumor resection and fewer associated surgical complicatio...

Management of Pituitary Adenomas: Mononostril Endoscopic Transsphenoidal Surgery

Introduction: The endoscopic transsphenoidal approach for pituitary adenomas and other sellar lesions is quickly becoming the procedure of choice in their surgical management. The most common approach is binostril three-hand technique which requires a large exposure and subjects both nasal cavities to potential trauma. To reduce nasal morbidity, we employ a mononostril two-hand technique with t...

Sellar reconstruction algorithm in endoscopic transsphenoidal pituitary surgery: experience with 240 cases

Background: Proposing a strategy for sellar reconstruction in endoscopic transsphenoidal transsellar approach for pituitary adenoma. Methods: 240 patients with pituitary adenoma underwent pure endoscopic endonasal transsphenoidal surgery. Intra-operative CSF leaks were classified as grade 0, no observable leak grade 1, CSF dripping through an arachnoid membrane defect of less than 1 ...

A new modified speculum guided single nostril technique for endoscopic transnasal transsphenoidal surgery: an analysis of nasal complications.

Abstract The endoscopic transnasal, transsphenoidal surgical technique for pituitary tumour excision has generally been regarded as a less invasive technique, ranging from single nostril to dual nostril techniques. We propose a single nostril technique using a modified nasal speculum as a preferred technique. We initially reviewed 25 patients who underwent pituitary tumour excision, via endosco...

A Prospective Study To Evaluate The Role Of Perioperatve Cerebrospinal Fluid Diversion By Lumbar Drain In Transnasal Transsphenoidal Tumor Surgeries

Background &Aim: The endoscopic endonasal transsphenoidal approach has been widely employed as a treatment for pituitary adenomas and other parasellar tumors. However, the major concerns related to endoscopic endonasal transsphenoidal surgery include the risks of CSF leak post-operatively leading to morbidity and at times mortality due to severe meningitis. Several studies in the past have foun...
